[Technological innovation goes ahead] The Department of Plastic Surgery of Shandong Provincial Hospital Affiliated to Shandong First Medical University (Shandong Provincial Hospital) successfully performed breast reconstruction for a patient.

Recently, under the guidance of Professor Huo Ran, Director of Plastic Surgery of Shandong Provincial Hospital Affiliated to Shandong First Medical University (Shandong Provincial Hospital) and Xu Guangqi, Deputy Chief Surgeon of Plastic Surgery, the team successfully performed breast reconstruction for a patient after right breast resection. The operation effect was good, the right breast shape was perfect, and the contralateral breast was basically symmetrical. At present, the technology takes the lead in Shandong province and the whole country.
Seven years ago, the patient had a strong desire for breast reconstruction because of right breast resection, with a surgical scar measuring 12 cm in diameter. After many consultations, she came to the Plastic Surgery Department of the hospital. After three-dimensional CT examination and evaluation, the patient met the surgical requirement and was admitted to the Department of Plastic Surgery. After the patient’s admission, Professor Huo Ran and Xu Guangqi, Deputy Chief Surgeon, attached great importance to the case, listened to patients' demands and answered patient’s questions. The nursing team assessed the patient’s condition, and guided her to have a preoperative nutritional diet and sufficient rest.
In order to help the patient reduce her psychological burden and ensure the effect of the operation, Xu Guangqi and the doctors of his team, under the guidance of Professor Huo Ran, formulated an individualized and precise surgical plan for the patient after comprehensive evaluation.
In April 2021, the patient was admitted to the hospital for one-stage lower abdominal flap transfer and right breast reconstruction. During the operation, the skin and fat of the lower abdominal wall were removed and the perforating vessels were preserved. The second and third costal cartilages on the right side were partially resected to expose the perforating branches of the internal thoracic artery, which were anastomosed with the vessels of the flap. The patient had received radiotherapy and chemotherapy after operation, so the quality of blood vessels was poor, which brought many difficulties to vascular anastomosis. Finally, the anastomosis was completed with the concerted efforts of the team, and the flap survived after operation.
In March 2022, the patient was admitted to the hospital again for the second stage of full breast reconstruction. Based on the first stage of operation, the reconstruction of nipple, areola, submammary fold, axillary fat tissue transplantation and axillary line plasty were completed, and the breast plasty and breast cosmetic subunit remodeling were completed. After the operation, the patient was in good condition, the incision healed well, and the size of bilateral breasts was symmetrical, which met the aesthetic standards of human body. On the right side of the reconstructed nipple, the flap survived well, and the bilateral breast volume, nipple and areola were basically symmetrical. The patient was very satisfied with the results of the operation.
Traditional radical mastectomy for breast cancer includes the whole breast and its surface skin, breast surrounding tissue, pectoralis major muscle, pectoralis minor muscle, axillary and subclavian fat, and lymphoid tissue. Obvious asymmetric deformity of the breast after surgery leaves a psychological burden on patients and also causes great distress to daily work and life, especially in young women, so the need for breast reconstruction is more urgent.
